  1. February 2026

    OpenAI banned the Tumbler Ridge shooter's account months before the attack for violent queries. No escalation to law enforcement was made. Eight people died. A lawsuit filed by a victim's family is now in discovery. The structural failure: detection worked, internal enforcement ran, the escalation handler was never built.

  2. April 2026

    The FSU shooter used ChatGPT to plan weapon selection, ammunition, and timing before the Florida State University shooting. OpenAI cooperated with investigators after the event. The Florida Attorney General opened a criminal investigation into OpenAI. The Morales family filed a wrongful-death lawsuit naming OpenAI as primary defendant. Source: cnn.com
